Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU 

This position requires applicants to successfully complete a background check through the New York State Office of Children and Family Services when it supports the mandate under Section 424-a of the Social Services Law for legally authorized agencies to inquire of the Statewide Central Register of Child Abuse and Maltreatment (SCR) as to the existence of any indicated reports of child abuse and maltreatment against an applicant prior to employment, certification, or licensure in the childcare field.  All background checks will be conducted and reviewed in accordance with applicable federal, state and local law. 

Compensation Range: $18/hr - $20/hr commensurate with experience.

Overview:

The Intake Specialist will be responsible for orienting new patients to the health center, conducting insurance verification, facilitating enrollment in insurance as needed, ensuring sliding fee scale status is up to date and accurate on a daily basis, conducting PRAPARE assessments, and assisting the Patient Care Coordinator with medical case management as needed.

Responsibilities: 

  • Completes intake screening of new patients and identify program eligibility to ADHC, OASAS, Health Home, OMH, PrEP, UND, HEP C, RAD etc.
  • Completes PRAPARE screening for new and annual patient visits
  • Generate lead in eICare for new patients including MRN
  • Complete sliding scale application for uninsured/underinsured patients and uploads into the EMR.
  • Assist with enrolling uninsured patients with insurance including ADAP applications for Health Services Clients.
  • Coordinates the enrollment of patients into the UND program
  • Provides assessment (PRAPARE) and referral services for all clients enrolled in PC, which include but is not limited to, coordination of care and case management referrals
  • Assesses primary care patients for entitlements and assist patients that need to enroll in Medicaid HMO. Assist clients with changing their insurance/ PCP.
  • Participates in interdisciplinary meetings, health fairs for insurance enrollment
  • Participates in Morning Huddle, regular medical staff meetings including Quality Improvement.
  • Documents clinical interventions in the PC medical records.
  • Performs outreach activities by telephone, formalized written letters to enrolled patients who have not accessed services or who have not shown up for appointments for more than three months. Evidence of outreach efforts must be documented in the medical record.
  • Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
  • Assist medical providers with all paperwork needed by clients in regard to various needs, i.e transportation, home health aide, verifications, etc. Complete SSI paperwork requested and send it to the appropriate place.
  • Assist with patient registration process with professionalism and accuracy
  • Performs professional and efficient scheduling of patient appointments
  • Understands the needs of our customers, effectively provides service and or resolves problems, escalate to Clinic Manager or Clinic Director. Dedicated to meeting the expectations of internal and external customers, working to create a positive experience for all
  • Understanding while working in a fast pace environment with competing priorities may require assisting in other roles as needed.
  • Other duties as assigned by either Clinic Director or Clinic Operations Manager
  • Ability to work occasional evenings and weekends with advance notice.

  • Excellent communication skills

  • Skilled in interpersonal relationships with diverse populations. 
  • Associates Degree preferred.
  • The candidate must have a strong knowledge in HIV/AIDS, case management and entitlements.
  • The candidate should be able to perform psychosocial assessments and reassessment and demonstrate the ability to discern medical emergencies and alert the appropriate medical and clinical personnel for assistance and support.
  • Knowledge of HIPAA regulations on requests for medical documents is required.
  • Strong organizational and clinical skills required
  • Proficiency in Spanish is preferred.
  • Candidate must be willing to work on weekends.
