Job Summary

The major purpose of the Early Intervention Case Manager position is to provide service coordination to infants and toddlers with developmental delays and their families through a comprehensive program of integrated services.  Case Managers assist families in linking with community resources while offering support and guidance in child development.  Case Managers serves as a bridge between families and providers, ensuring that the families’ needs are being met, the coaching model is being observed and providers are working within the child's natural environment.  The Case Manager ensures that providers have the support and the documentation required to serve the child and family.  The Case Manager ensures that there is a plan in place for the family before the child ages out of the Early Intervention program.   

Essential Functions

  1. Participate as an Individualized Family Service Plan (IFSP) team member to determine initial eligibility for federally mandated Part C Early Intervention Services.  The Case Manager may participate in the development of the Initial IFSP development and is responsible for monitoring and revising the IFSP, ensuring Procedural Safeguards.
  2. Oversee maintenance of the IFSP to include collaboration of services, secure progress notes from providers and medical records from physicians, ensure Procedural Safeguards, and utilize the IFSP review process when adjustments to services are needed.  The Case Manager facilitates the transition process when Early Intervention Services are no longer appropriate for the client.
  3. Ensure that entitled services are delivered at the frequency/intensity specified on the IFSP.  The Case Manager also ensures compliance with federally mandated timelines including entitled services starting within thirty (30) days and obtaining a physician signature for any initial services, revisions to the IFSP, and annual IFSP development.
  4. Assist families with identifying, locating, and securing community resources.
  5. Ensure accurate and current documentation of home visits for Case Manger as well as collateral contacts, such as phone calls, written correspondence, and direct communication with families.
  6. Submit paperwork and reporting information within required timelines to include Credible services, child data forms, time sheets, and other data as requested by supervisor.
  7. Ensure that the delivery of services occurs in the child's natural environment as defined and mandated under federal law and determined on the IFSP.  Because services are provided in a natural environment, extensive travel may be required.
  8. Maintain a professional and supportive relationship with families and providers. Ensuring the providers have the documentation to perform the entitled services within federally mandated timelines. Assist families and providers in understanding and participating in the coaching model for entitled services. 

Experience/Education Required

Requirements and Process for Initial Certification as an Early Intervention Case Manager:

Applicant must hold:

 • A minimum of a bachelor’s degree in any of the following fields: 

  • Allied health, including rehabilitation counseling, recreation therapy, occupational therapy, physical therapy, or speech or language pathology.
  • Child and family studies.
  • Counseling.  
  • Early childhood.
  • Early childhood growth and development.
  • Early childhood special education.
  • Human development. 
  • Human services. 
  • Music therapy. 
  • Nursing. 
  • Psychology. 
  • Public health. 
  • Social work. 
  • Special education – hearing impairments. 
  • Special education – visual impairments.
  • Other related field or interdisciplinary studies approved by the State Lead Agency;

Valid Driver’s License and copy of DMV safe driving record is required
