The Medical Social Worker is a skilled and experience professional Social Worker responsible for excellence in clinical practice of social work. Reporting to the Director of Ambulatory Services and the Social and Community Services Supervisor, the Medical Social Worker is responsible for providing a range of psychosocial assessments, therapeutic interventions, crisis intervention, short term case management, providing information and connecting resources for our patients and/or families. The role reflects appropriate knowledge of Social Work scope of practice, current state requirements, CMS Conditions of Participation, The Patient Bill of Rights, and other Federal or State regulatory agency. The Medical Social Worker partners with the medical staff and utilizes relevant data to manage the care of the patient. These activities include seeing the patient in the ambulatory setting, assure high quality of care and efficient utilization of available healthcare resources, facilities and services and Health Plan reimbursement categories. This position requires the full understanding and active participation in fulfilling the Mission of Martin Luther King, Jr. Community Hospital. It is expected that the employee will demonstrate behavior consistent with the Core Values. The Watson Model of Care is utilized throughout the delivery system of care services.



  • Is responsible in assessing, planning, managing, and providing social services in a medical setting for patients and/or their families including ambulatory medical setting.
  • Develops a treatment plan or plan of care which includes arrangements for referral to services as appropriate.
  • Provides community resource information and performs referral and liaison activities as necessary for the patient’s ambulatory care.
  • Acts as a consultant to patients requiring information on Advanced Directives and/or end of life planning.
  • Participates on an assigned basis in educational training for employees and providers related to the psychosocial aspects of physical and emotional illness/disability.
  • Acts as a resource to other Social Workers within the organization on difficult to manage cases.
  • Maintains current information on all frequently used community and county based resources.
  • Assisting in routinely updating resource list and is available to discuss how to resource required services.
  • Identifies and intervenes with high-risk patients requiring social services through all ambulatory case management programs.
  • Assist providers and care managers by acting as a consultant for patients with complex psychosocial needs.
  • Collaborates with the interdisciplinary team; participants in team meetings, huddles, etc to facilitate timely care; assure quality of care throughout the care of the patient; and minimize adverse outcomes.
  • Provides counseling and crisis interventions to patients and/or families over issues related to psychosocial issues.
  • Prioritizes cases and responds to priority system appropriately.
  • Communicates relevant elements of the health plan benefits.
  • Documents all team, physician and patient/family communication and concerns pertaining to coordination of care and services.
  • Screens every patient to identify need for further assessment of medical necessity (standard description of which patients are seen).
  • Adheres to the Ambulatory Department policies and procedures.
  • Considers the patient population served, age-specific criteria and the Watson Model of Care in all patient/family care and interaction.
  • Other duties as assigned.




A. Education

  • Bachelor’s in Social Work required. Master’s degree in Social Work preferred.
  • Candidate's with a Bachelor’s degree in a related field and with at least one (1) year of experience in social work/social services.

B. Qualifications/Experience

  • Minimum of one (1) year of experience in social work, social services, or similar role. Social Work internships will be considered as experience.
  • Three (3) years of experience in a hospital or other healthcare setting preferred.
  • Experience in working with the underserved population
  • Knowledge and understanding of clinical social work and mental health counseling principles, methods, procedures, and standards.
  • Knowledge of clinical operations and procedures.
  • Able to navigate and connect successfully with outside provider networks (Health Plans, IPA’s, and FQHC’s).

C. Special Skills/Knowledge

  • Bilingual language skills preferred (Spanish)
  • Current Basic Life Support (BLS)
  • Must complete annual Workplace Violence Prevention Program/Certificate, per hospital policy, during initial training/orientation but not to exceed 30 days from hire/transfer.
