The Home Health and Hospice RN is involved in planning, organizing, and directing home health and hospice services. This includes the coordinating of community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.

  • Comprehensive assessment of patient and family to determine home care needs. 
  • Provides a complete physical assessment and history of current and past illness. 
  • Coordinates appropriate care planning with patient and family. 
  • Continuously works toward discharge planning needs and goals with patient, family, and interdisciplinary team. 
  • Effectively communicates patient and family needs to interdisciplinary team. 

Minimum Requirements



You would be a great addition to our team if you possess the following:

  • Degree from an accredited school of nursing.
  • Current RN licensure to practice professional nursing in the state.
  • Three years of experience in a home health/hospice setting.
  • Valid driver's license  with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Excellent observation, verbal, and written communication skills.
  • CPR certification.
