POSITION SUMMARY

Reporting to the Director of Nursing of  MLK Community Medical Group, the Ambulatory Clinical Triage RN provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. This position supports MLKCH’s outpatient site(s) and ambulatory services involving one or more specialty disciplines, and demonstrates performance consistent with the MLKCH Vision, Mission, and Strategic Plan of the organization. Within assigned area, this position will implements supervise/monitor, maintain clinical practice and quality management standards, as well as staff competencies and development.  The Ambulatory Clinical Triage RN is to remain flexible to changing systems, is expected to demonstrate quality and effectiveness in work habits and clinical practice and treats co-workers, patients, families, and all members of the health care team with dignity and respect

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Serves as a leader of the health care team; delegates tasks appropriately, and demonstrates appropriate accountability.
  2. Prioritizes, delegates, and supervises clinical practices for completion of patient care activities.
  3. Upholds highest quality of care standards to comply with the Nurse Practice Act, TJC, federal/state/local requirements.
  4. Performs detailed evaluation of patient symptoms and determines urgency of patient condition, facilitates next steps in patient care, makes recommendations for escalation of care, and communicates changes in patient condition to health care provider.
  5. Provide telephone triage for same day appointments.
  6. Functions as a contact for providers, staff, and patients in order to address problems, issues or concerns that require attention.
  7. Performs timely research and resolution of patient grievances.
  8. Establishes and maintains effective, collaborative relationships with physicians and other departments.
  9. Monitors provider message inboxes and processes medication refill requests and other administrative tasks as appropriate within scope of practice.
  10. Collaborates with nursing and other departments to resolve patient care issues, meet access demands, provide convenient hours of operation, improve the cost structure and develop consistent policies and procedures.
  11. Upholds highest quality service standards to meet members' and internal clients' expectations.
  12. Acts as patient advocate and resolves patient care issues.
  13. Recommends and evaluates processes to improve systems and patient care results across the continuum of care.
  14. Identifies process efficiencies and recommends strategies to reduce costs and improve quality of care/service.
  15. Oversees use and maintenance of equipment, supplies and medications.
  16. Assists Site Administrator(s) in developing and maintaining the competency of all department staff.
  17. Collaborates with physicians, Medical Director, Site Administrator(s), VP of Ambulatory Services,  in developing the appropriate level of patient services and the day-to-day operations of the department.
  18. Measuring and improving internal and external customer satisfaction.
  19. Assists in resolving department safety, and risk management issues.
  20. Procure supplies and other materials needed for the medical group site(s) at competitive prices and with reputable vendors.
  21. Participate in development and implementation of long-range plans.
  22. Assist in resolution of administrative and operational issues including the maintenance of medical group property, computer systems and installed software applications.
  23. Serve as a liaison between site and external agencies.
  24. Assists leadership  develop medical group strategic plans and objectives based upon identified needs of patients.
  25. Maintain the strictest confidentiality.
  26. Utilizes communication strategies including chain of command and issue escalation, which result in intended outcomes.  
  27. Mentors, orients, and coaches others in unit specific operations and patient care activities, shares responsibility and authority with subordinates and is accountable for performance.
  28. Works with other departments in order to assist patient problem resolution.
  29. Monitors the patient progress based on the care plan, revises the care plan based on condition and evaluation of progress.
  30. Assists with development and completion of discharge plans including addressing discharge barriers and patient/family education.
  31. Helps with patient understanding of medication purpose, side effects, and administration instructions.
  32. Makes referrals for appropriate resources and facilitates patient understanding of utilization of resources
  33. Perform other duties as assigned.
  

A.   Education

  • Bachelor’s degree in Nursing preferred.
  • Current California RN license.
  • Current Basic Life Support for Providers, by American Heart Association (AHA)
  • Advanced Life Support certification (ACLS), by American Heart Association (AHA)

 

B.  Qualifications/Experience

  • Registered Nurse with one (1) year of ambulatory clinical experience required, two (2) years of clinical experience preferred in a inpatient or outpatient setting, within the last 5 years.
  • Bilingual in Spanish and English required
  • Critical care experience preferred
  • Telephone triage experience preferred
  • Has excellent observation, verbal and written communication skills.
  • Highly organized and ability to work independently.
  • Ability to work with people of diverse backgrounds.
  • Computer literate with proficiency in Microsoft Office, Google Suite, electronic medical records and ability to learn new applications easily.
  • Demonstrates ability to supervise and direct administrative personnel.
