SUMMARY:The Community Health Worker is responsible to be a group facilitator, peer to peer escort to community appointments and resources, assists with case finding and re- engagement services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Be a member of an individual treatment team for identified clients enrolled in mental health and substance use counseling services.

2. Assist with outreach and recruitment of new clients at health fairs, community forums and other activities.

3. Provide wellness support services to individuals, families and groups, biomedical counseling to clients and partners, provide client assistance in the form of community accompaniments, as well as outreach for engagement and re- engagement.

4. Provide supportive services such as assistance securing benefits and entitlements help with independent living skills including self-care, living situations, nutrition, money management.

5. Be able to identify substance abuse history, triggers, and clients past treatment history and coordinate services with the treatment team.

6. Be able to document client’s substance abuse symptoms and clients responses to treatment. Share observations and concerns with the team for consultation and direction and take appropriate action.

7. Work under the Risk Reduction Model of care.

8. Maintains program in contractual compliance by documenting movement of treatment for the client in progress notes and completing reports for funders, and utilizing existing program forms to gather client data. Community Health Worker

9. The Community Health Worker will be expected to sit for trainings both within Bailey House and outside of Bailey House further knowledge of working with substance users and people living with HIV/AIDS as required by the funder.

10. Maintains deadlines, timeframes and other related job functions as discussed by supervisor

11. Enhance BHH services by providing information and referrals to PWAs and other service providers.

12. Perform the essential functions of the position.

13. Follow agency policies and procedures.

14. Maintain professional and technical knowledge.

15. Understand and comply with agency guidelines in the personnel handbook.

16. Perform other responsibilities and duties as assigned.

MINIMUM EDUCATION/EXPERIENCE REQUIREMENTS:

1. The Community Health Worker may have been trained by an organization that offers peer education services, have a high school diploma and/or life experience. Must have some experience working with individuals living with HIV/AIDS

2. Must have completed or be willing to complete within 6 months of employment, AIDS Institute Certified Peer Worker training.

3. The Community Health Worker must be able to communicate orally and inwriting. Bilingual (English and Spanish) preferred.

4. Must be able to work cooperatively and productively in an interdisciplinary group of professionals.

5. Must be flexible and able to work with a homeless and under-housed, often substance using population.

HIPAA: The Community Health Worker will have access to PHI during the course of his/her work activities. The Community Health Worker will use this information to monitor and supervise the care coordination that will occur during the patient’s visits with providers, care managers, PNs, and to monitor the provision of core services and implementation of the care plan. ALL HIPPA , corporate Community Health Worker compliance, and Health Home guidelines must be maintained and kept at all times.

PHYSICAL DEMANDS/WORKING CONDITIONS:

1. Must be able to remain in stationary position 50% of time. Frequently move about inside the office to access file cabinets, office machinery, meet with clients, supervisor or co-workers etc.

2. Constantly operates a computer and other office productivity machinery such as calculator, copy machine and computer printer

3. Frequently communicates (exchanges information, converse with, express oneself to) with subordinates, vendors, supervisors, community at large, clients regarding treatment, management, incidents, reporting, advertise services.

4. Must be able to determine accuracy of reports (figures, balances, documentation) perceive risk, agitation of clients, judge, observe and assess situations germane to clinic operations.

5. Excellent verbal and written communications skills; articulation and instruction of policy procedure and practice. Coach staff in accordance with agency policy and best practice.

6. Frequently exchange information in communication with individuals who are agitated, confrontational or difficult patients and staff from a variety of socio- economic, cultural and religious background.

7. Occasionally move, position or put files weighing up to 10lbs.

8. Occasionally required to traverse public transportation in order to attend meetings, give presentations or meet with clients in community.

9. Noise level is moderate

10. In the office 70-80% of the time. May need to work remotely and access computer software. Frequently need to manage, manipulate or maneuver use of technological devices.

11. Occasional exposure to inclement weather when conducting field work.

12. Frequently travel (20-30%) of time to unsafe neighborhoods, traverse public transportation in accompaniment of client or to network/build relationships with community partners.

13. Occasional to frequent exposure to patients with infectious diseases.

Salary range* 33k-36k

This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”).  Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU.
