Position Overview

Participate in targeted outreach activitiesthrough both traditional and new media outreach to link interested individualsto servicesat Housing Works as indicated. Collaboratewithother service providers by utilizing technology such as Outlook 365 and ECW (electronic medical record system). The Outreach Specialist will also remindclients of appointments, follow up on missed appointment, reschedule missed appointments if applicable, and monitor continuedusageofservices through use of tracking systems. They will also serve in the capacity of back up phlebotomist and provide STI and HIV testing as needed.



Participate in targeted outreach of the key population (MSM of color and transgender women) for HIV and STI testing and engagement in prevention services Ensure at risk and newly diagnosed clients are linked to Primary Care for confirmatory testing, STI screenings and PrEP/PEP when applicable Serve in the capacity of backup phlebotomist during weekly drop in and testing events Conduct Risk Screening, Brief Intervention, Intake Assessment and assist with case management activities and integrated case conferences when requested Conduct follow up outreach with linked clients via various methods (in person, phone, social media, new media, etc) to increase consistent engagement in services, such as primary care, mental health, substance use, etc Participate in the implementation of the new media plan, including upkeep of targeted dating site professional profiles and social media posting across various platforms as part of the strategic social media plan Facilitate the client experience from PrEP outreach to follow up by providing basic education regarding PrEP and PEP, scheduling appointments in ECW, communicating appointments to medical staff, and retention services Record and track data through multiple database systems such as ECW, AIRS, Eicare, and Eshare Engage in weekly supervision with the Program Coordinator Participate in reoccurring interagency meetings, as well as external community meetings when applicable Perform other duties as assigned

Minimum Requirements


Educational Requirements

Demonstrated experience working with MSM/Transgender communities of color Demonstrated understanding of PrEP/PEP and HIV treatment in communities with barriers to accessing care Strong socio-cultural identification with the priority populations of this service category Strong organizational skills and verbal/written communications skills Ability to work independently with minimal supervision Organizational and multitasking skills to work in a fast paced environment are a must Knowledge ofissues pertaining to homelessness, substance use, harm reduction, mental illness,transgender healthcare, and LGBT youth Experience in utilizing technology such as Microsoft Office tools, Callrail, Eicare, Eshare, ECW, texting, emailing Strong customer service skills over the phone and via electronic communication Willingness to participate in occasional advocacy events around the city Bachelor’s Degree or Associates preferred, High School Education required Bilingual preferred Previous certification in phlebotomy preferred History in sales preferred Ability to work some early evenings regularly, M-F as well as occasional weekend events
