Position Overview

The Veteran's Housing case manager promotes independent living within the Veteran's Housing Program by providing structured case management services aimed at developing and enhancing skills of clients within the program.

 

Responsibilities

Case load of 25 to 30. Facilitate support groups. Link Clients to health care, benefits and entitlements within the community. Escort clients to appointments. Implement service plan for caseload. Maintain agency standards and regulatory/funding mandates.

Educational Requirements

Must Possess a BSW or Equivalent degree. Experience with homeless chronically ill, MICA, and formerly incarcerated and/or providing service to military Veterans.
