Position Overview

Family Case Manager promotes independent living within the family services program at Schafer Hall by providing structured case management services aimed at developing and enhancing skills of clients and their families within the program.



Maintain program in contractual compliance by completing assessments, service plans, progress notes and other required forms on time. Carries case load of 20 to 25. Keeps contact with clients including monthly home visits. Assists clients with budgeting, shopping counseling regarding rent arrears, providing ADL training, referrals to support groups, providing new clients with a program structure. Provides crisis intervention and mediation as necessary.

Educational Requirements

BA or BSW with at least 2 years of case management or related experience with individuals living with HIV/AIDS.
