As a Case Manager, you will be responsible for providing ongoing support and resource coordination to families struggling with issues related to poverty, homelessness, child welfare engagement, mental health and/or parenting. You will assist families in connecting with resources in the community, basic needs, and work to accomplish client determined goals and enhancing participant’s knowledge of family safety, parenting and life skills, and relative caregiving through groups, classes, workshops, resources, referrals, and outreach or individual consultation. This program’s focus is to provide case management services in Ramsey and Hennepin County to families.

This role will work with elementary students to support and reduce truancy.

Minimum Requirements

  • Bachelor’s Degree in Social Work or related field required. 
  • Two years of relevant experience with families struggling with issues such as poverty, homelessness, addiction, domestic violence or child welfare concerns are strongly preferred.
  • A combination of six years of relevant experience, education or training working with families and especially with relative caregivers is considered.
  • A driver’s license, acceptable driving record and current insurance is required.
  • Able to drive a 12 passenger agency van.
  • Strong communication skills to employ knowledge of family functioning including building rapport and trust with each family.
  • Able to empathize and connect with low-income families, teen parents, grandparents and others raising relative’s children and to work effectively with participants of diverse backgrounds.
  • Able to function independently and interact with and relate to a variety of individuals.
  • Computer skills in the areas of word processing, database management, and publishing programs
