Summary Description:
It is the responsibility of the Healthcare Integration Manager to manage a team of Intake Coordinators ensuring adherence to policies, procedures, drive positive and measurable outcomes. Intake Coordinators are responsible for enrolling members at internal and external healthcare settings. The Integration Manager is an exciting opportunity to build relationships with external medical providers/hospitals, and assist in creating strategies to drive program growth and expansion for Housing Works integrated care management services.

• Provides direct leadership and management for the Intake Coordinator team.
• Collects, monitors, and analyzes enrollment operations performance data relative to established goals and metrics to identify potential risks and/or areas of opportunity and prepares action plans for improvement.
• Manage productivity based on department metrics to ensure workloads are being handled in an efficient and productive manner.
• Support the administrative needs of Intake Coordinators, including but not limited to prescreening eligible members and processing intakes.
• Performs other duties as requested.
• Achieve set Key Performance Indicators (KPIs).
• Formulate and implement rapid change and process improvements.

Minimum Requirements



• Master’s degree Human Services or related field preferred.
• Experience in marketing healthcare related products to consumers.
• Minimum of 2 years experience in health/Medicaid Managed Care or related healthcare insurance enrollment.
• Supervisory experience is required
• Computer literacy, including Microsoft Office
