As a Community Health Worker you will provide one-to-one support to older adults who participate in the Community Companion program through healthcare partnerships with Lutheran Social Service of Minnesota.

This program’s mission is to support the older adult’s transition to life at home after a hospital stay. Major objectives are to increase the participant’s access to needed community resources and to reduce addition emergency-room visits and hospitalizations.  You will do this by:

  • Scheduling in-home and phone visits
  • Assisting the program participants in understanding discharge orders and completing his/her personal health record
  • Conducting additional appropriate assessments, including in-home safety assessment and use of meal delivery service
  • Referring program participants to additional services and other community resources that promote self-care skills and increased self-sufficiency in managing his/her healthcare.

Minimum Requirements

  • Completion of Community Health Worker certificate program at an accredited Minnesota institution.
  • 0-1 year experience in healthcare and/or older adult service preferred.
  • Ability to work with individuals and families from diverse backgrounds with care and compassion.
  • Knowledge of resources available in program participant’s community preferred.
  • A valid driver’s license is required.
