Housing Works Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs.

Summary Description:
The Intake Coordinator is responsible for adhering to the agencies intake procedures and ensures assessment activities are completed within specified time frames. Perform all or some of the following functions: initial intake, assessment of client’s complexity level (e.g. Low Need/ Medium Need/ High Need), make referrals, verifies Medicaid entitlements, and request medical documentation. When appropriate, provide treatment recommendations within the Housing Works network of social services, behavioral health, primary care, and treatment programs. If caller/potential client is not appropriate for Health Homes or the Adult Health Care Program (ADHC), provide internal referrals based on presenting issues. Identify trends that impact the intake process, workflow, and track the retention rate among newly enrolled clients. Performance is measured by the ability to track changes to the client’s complexity levels, monitor retention rate of newly enrolled clients, and the ability to enter intakes within specified time frames.

1. Determine client’s medical/insurance eligibility for the Health Home Program and the screen appropriate candidates for services at Housing Works Adult Health Care Program (ADHC), and Primary Care Services.
2. Coordinate intake appointments and referrals with Housing Works Community Partnerships Department. Work with the Borough Directors and Unit Supervisors to ensure adequate intake coverage in the office and field.
3. Collaborate with the Data Manager to ensure that potential clients are not dually enrolled in a Health Home outside of Housing Works.
4. Identify and assess an individual’s current situation, issues and needs. Determine the most appropriate clinical interventions for helping the individual achieve measurable clinical outcomes. Clients should be assessed and/or appointments made for intake and assessments during the first point of contact.
5. Obtain client consents (e.g. HIPPA, BxRHIO, Healthix, etc.) and schedule medical appointments (internal PC services) as needed.
6. Utilize the electronic health system to run reports (60 days after intake) to verify submission of medical documentation among newly enrolled Health Home clients.
7. Run monthly reports to ensure that the client’s complexity level has been re-assessed by Care Managers.
8. Run monthly reports to identify the client retention rate (e.g. retention rate at 90 days, 120 days, and 180 days after intake date) among for newly enrolled clients.
9. Establish and maintain cross-departmental professional relationships within Housing Works Primary Care Department, ADHC Program, and OASIS Programs.
10. Provides back up coverage to ensure that the coordination of Health Home services are available.
11. Other duties as assigned

Minimum Requirements


1. Master’s Degree Preferred (MSW, LMSW, etc.) and Bachelor’s Degree in a health related field is required.
2. Minimum of three years of case management experience in the medical field or social service sector.
3. Must have an understanding of treatment modalities, levels of care and experience with mental health.
4. Experience with coordinating health related activities, utilization review, identifying health trends, and analyzing health related data.
5. Demonstrated capability to gain commitment, build rapport with others required.
6. Maintain professional and positive attitude when working with callers/potential patients, members of the call center team and facility Intake Departments.
7. Knowledge of managed care, Medicare/Medicaid and insurance authorization/billing is a plus.
