Why Housing Works 

Housing Works was founded in 1990; With a long-term commitment to AIDS advocacy. Housing Works established New York State’s first harm reduction-based, OASAS-licensed outpatient drug treatment program. Other services include Health Home care management, behavioral health, and syringe exchange programs. We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. 

Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. 

We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and textbooks. 

Position Overview  

Care Navigators at Housing Works are essential members of the Case Management team. As entry level human service professionals, Navigators assist Case Managers with providing care coordination services to clients living with chronic illness, homelessness, substance use and/or mental health issues. 


Services provided include scheduling client medical appointments, conducting home visits, providing basic health education, and other services to help clients navigate the healthcare and public benefits system.

This position is ideal for new or recent bachelor’s level professionals who are committed to social justice and looking to gain the skills and experience needed for a career in human services.
