This position is full-time, nonexempt, classified and Covered as defined by The Board of Commissioners of the County of Allen Employee Handbook.

Hours are 1:00 pm - 9:30 pm; Monday - Friday; 40 hours/week; overtime as required

Under the direction of the Social Services Coordinator, the Case Manager supports administration through the maintenance of confidential resident records, supports Youth Care Workers through assistance of resident supervision, provides counseling of residents and communicates with all parties involved in the resident cases including the Department of Children’s Services (DCS), Juvenile Probation, the parents/guardians and service providers.  The Case Manager assess each resident on their risks and needs by completing required assessments, develops a plan for care and assists staff in implementing each plan.

ESSENTIAL FUNCTIONS:

  • Completes Child and Adolescent Needs and Strengths (CANS) assessments and evaluates Outcomes Now (EON) and other diagnostic tools to determine the level of functioning for each resident.
  • Counsels and supervises residents in an emergency shelter care facility, putting a high priority on safety and security. 
  • Maintains confidentiality with resident files and databases for current and past residents and documents behaviors on various forms and incident reports. 
  • Maintains all Performance Quality Improvement (PQI) and Environment of Care (EOC) documentation per state requirements and facilitates quarterly reviews to identify opportunities for growth and review critical incidents that have occurred.
  • Completes admissions and intake process, develops care and discharge plans and assists in maintaining reports held within Microsoft Access, including the daily census of residents.
  • Tracks all resident diagnoses and length of stay and provides statistical analysis of the diagnoses of residents served at the facility.
  • Completes monthly reports for the National Youth in Transitions Database to track independent living skills training taught to residents age 16 and older and provides statistical analysis of staff submissions of trainings completed.
  • Counsels residents to help identify coping skills, engage in active listening to process their stay at the facility and the trauma they’ve endured and assist with conflict management amongst residents.
  • Communicates with parents/guardians, Department of Children’s Services (DCS), juvenile probation, therapists, service providers, etc.
  • Assists with transportation for residents to and from appointments, court hearings, etc.
  • Supervises residents on the unit and assist the Youth Care Workers with activities including, but not limited to, laundry, medication passes, restraints and crisis management, room searches, incident reporting, etc.
  • Supervises visitation hours, answers phone calls, updates resident case notes, supervises the front office and assists in a variety of front office management after normal business hours.
  • Responds to emergencies within the facility including, but not limited to conflicts between residents where physical restraint may be required, runaways, suicidal youth, dangerous weather and medical issues.
  • Performs all other duties as assigned, including overtime as required.

 

Minimum Requirements and Working Conditions

 

REQUIREMENTS:

  • Master’s Degree in Social Work, Psychology or Counseling from an accredited school 
  • At least one year experience working in social services, psychology, or mental health field
  • Valid certification in Therapeutic Crisis Intervention
  • Valid certification in CPR and First Aid
  • Valid Driver’s License to operate a County vehicle for transporting residents to and from court and various appointments
  • Valid CANS Super User certification within 90 days of employment
  • Ability to maintain strict confidentiality
  • Strong computer skills with experience in Microsoft Office, Access, Quest, as well as CANS and EON applications
  • Critical thinking skills and sound judgment 
  • Trained in Therapeutic Crisis Intervention (TCI) and able to use it effectively, specifically when assisting Youth Care Workers (YCW) in dealing with at-risk and challenging youth

WORKING CONDITIONS:

The Case Manager works in an emergency shelter care facility supervising neglected, abused and potentially troubled youth requiring the ability to be able to physically restrain residents, stand and walk frequently with some sitting, lifting up to sixty pounds, bending, pushing and/or pulling loads and reaching over head required.  The Case Manager must be able to very frequently type including transcription, proofreading and attention to detail, monitoring of equipment and detailed inspection is required.
