The Intake Specialist will be responsible for the screening of new patients including insurance verification, new registration, insure daily sliding fee completions and uploading in EMR. This will also include completion of required documentation in the EMR.


• Completes intake screening of new patients and identify program eligibility to Health Home • Completes PRAPARE screening for new patient visits • Generate lead in eICare for new patients including MRN • Provides assessment (PRAPARE) and initial planning of care for all clients enrolled • Participates in interdisciplinary meetings, health fairs for insurance enrollment • Participates in Morning Huddle • Performs outreach activities by telephone • Evidence of outreach efforts must be documented in the medical record. • Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS, etc. • Assist health home staff with all paperwork needed by clients in regard to various needs, i.e transportation, home health aide, verifications, etc. • Assist with patient registration process with professionalism and accuracy • Performs professional and efficient scheduling of patient appointments • Understands the needs of our customers, effectively provides service and or resolves problems, escalate to Care Manager or Health Home management. Dedicated to meeting the expectations of internal and external customers, working to create a positive experience for all • Understanding while working in a fast pace environment with competing priorities may require assisting in other roles as needed. • Other duties as assigned by Health Home management • Ability to work occasional evenings and weekends with advance notice. • Understanding and practice of the Housing Works core values of All-In, Stronger Together, High Performance, and Membership


Minimum Requirements


Bachelors Degree required. The candidate must have a strong knowledge in HIV/AIDS, case management and entitlements. The candidate should be able to perform comprehensive assessments and reassessment and demonstrate the ability to discern medical emergencies and alert the appropriate medical and clinical personnel for assistance and support. Knowledge of HIPAA regulations on requests for medical documents is required. Strong organizational and clinical skills required and proficiency in Spanish is preferred. Candidate must be able to work on weekends if needed.
