The Engagement Specialist is a community field based position.  The Engagement Specialists conduct field work in the community and focus on reengaging individuals that need case management services.   The Engagement Specialist guides clients through the health care system by assisting with access challenges, develop relationships with service providers, and tracking interventions and outcomes.

  • Responsible for alerting or bringing important follow-up items to the Supervisor of Care Management.
  • 70-80% of time spent con ducting outreach activities in the community, such as visiting shelters, hospitals, etc.
  • Complete clinical documentation (e.g. write progress notes, care plans, etc.) .
  • Tack medical, behavioral, and support service referrals made for client and ensure that the client follows up and receives all of the care they need.

Minimum Requirements


Qualifications: A Bachelor of Arts or Science degree with two years of relevant experience or a Master’s degree with one year of relevant experience. • Excellent interpersonal, public speaking, and written communication skills. • Ability to work autonomously. • Demonstrate professionalism, respect and ability to work in a team environment

Salary: $42k
