Position Overview 

The Retention & Adherence Supervisor is responsible for general oversight of service delivery within the Retention & Adherence Department (RAD) at one of three clinic sites (East New York Health Center, Downtown Brooklyn Health Center, or Cylar House Community Health Center). He/she will provide oversight of all staff assigned to the CCR, MHV, and RAP (East New York only) contracts at assigned location, as well as assessment and screening of new clients enrolling in the various contracts. Additionally, the Retention & Adherence Supervisor will be the primary individual responsible for oversight of services provided to clients by his/her team, and he/she will provide direct therapeutic service and maintain a caseload of individuals receiving mental health services through the MHV grant. 

  

Responsibilities 

1. Responsible for monitoring the service-based aspects contract along with delivery of direct services.  

2. Provides direct services which include care coordination, treatment adherence counseling, and crisis intervention. Have experience in different interventions which include, but are not limited to; psychodynamic, cognitive behavioral, harm reduction, strength’s based, motivational interviewing and mindful-based practices. 

 3. Comply with the policies and procedures in compliance with NYS law and HW agency wide policies and procedures. These policies and procedures include those related to program operations, care and treatment of patients, service data collection and verification, medical billing, quality assurance, and handling of medical records.  

4. Ensures all client records are maintained in accordance with all state and federal laws, rules and regulations.  

5. Monitors programs’ enrollment and discharge trends to ensure retention and quality. Also monitors staff scheduling to ensure adequate program coverage at all times, as well as program financial trends to ensure targets are met at the site level.  

6. Manage public relations at the site level to promote awareness and understanding of available services within the RAD.  

7. Comply in the development and execution of Quality Improvement Initiatives in collaboration with agency’s QI Team.  

8. Maintains confidentiality and professional ethics.  

9. Reviews/approves assessments, reassessments, care plans, PCSMs, adherence assessments, and case conferences.  

10. Participates and supports advocacy efforts to end the twin crises of homelessness and AIDS.  

11. Provides regular supervision (individual and group) to all supervisees to ensure quality service delivery and treatment. 

Minimum Requirements

 

Job Requirements: 

Educational Requirements 

• Graduate degree in Social Work and is licensed in NYS as a Social Worker; LMSW required, LCSW preferred

• Five years or more of clinical experience in a health, mental health and/or social service setting. Experience operating a service based contract a plus, with at least two years of supervisory experience in a similar setting.
