Description

 

The Data/Program Associate will be responsible for performing an array of activities and projects intended to ensure the accuracy and completeness of process and outcome of program data and information collected for funder reporting, continuous quality improvement, outcome monitoring and audit purposes. This position provides data entry and data management support for the Bailey House Health Home program. The associate will use several web-based programs and electronic health records, including, but not limited to: Relevant Health Homes and eiCARE, as well as internal databases. 

Responsibilities:

  • Reviews data for completeness and accuracy in the EHR and internal databases.
  • Submits weekly reports as directed by the Senior Director of Health Homes.
  • Assists in the design of measurement tools, databases and data management systems.
  • Assists with data analysis and preparing evaluation reports to maintain contractual compliance with relevant funding sources.
  • Assists with data collection and analysis using all databases.
  • Assists with creating, running, and maintaining reports in all databases.
  • Collaboratively develops, refines, implements, analyzes and reports on aggregate data and program outcomes.
  • Performs other responsibilities and duties as assigned by Senior Director, Quality Assurance Specialist, Clinical Director, and Vice President, Community Program.
     

Minimum Requirements

 
  • BA/BS in relevant field
  • Experience in data entry (essential) and database development, data management and analysis (preferred).
  • Experience working with community-based organizations as support staff, evaluator, direct service provider or consultant.
  • Experience providing training and/or technical assistance.
  • Bilingual (English- Spanish) is preferred.
  • Knowledge of HIV/AIDS and/or other chronic diseases/disease management
