Description

 

Responsible for the overall administration of the HH teams as assigned. This includes clinical supervision of HH teams, coordination of client services, staff recruitment and hiring, staff training, community and case finding, weekly, monthly and quarterly reporting of HH information to the Director, and implementation of the program policies and procedures. Works with the Outreach Supervisor for delegation of rosters, monitoring and reporting of the tracking sheet and DOH matrix requirements.


Responsibilities:

  • Assist the Director with oversee the day to day operations of the Care Management department.
  • Supervise the Care Managers and Patient Navigators teams.
  • Provide supervision and monitoring of Care Management activities. Complete performance evaluation on newly hired employee and once a year. Complete corrective actions and follow-up activities.
  • Conduct client record reviews and maintains required data collection system.
  • Complete monthly report and submit to Director of Health Home Services.
  • Provides at a minimum supervision with the care management staff.
  • Assist with facilitating the preparation of staff schedules and ensure documentation of such in the EMR.
  • Works in conjunction with the Outreach Supervisor to assign patients to case management teams.
  • Ensures weekly, monthly and quarterly matrix and reporting are met and maintained.
  • Reviews and approves all assessments, reassessments and care plans.
  • Ensures rosters are updated on a daily basis as appropriate and ensures these rosters are kept, maintained, and monitored via the portal.
  •  Conducts monthly case record reviews to ensure quality service delivery and compliance with program standards regarding appropriateness of documentation.  
  • Identifies on-going case management staff training needs.
  • Assumes initiative in agency activities and seeks new sources of patient referrals and programs for the growth of the HH Program.
  • Completes probationary and annual performance reviews/evaluations for supervised staff.
  • Submits monthly tracking Reports to the Program Director.
  • Ensure that team complete core services for clients in a monthly basis.
  • Responsible for quality assurance and continue quality improvement (CQI) tasks.

Ensure compliance with:

  • Performance evaluations
  • Agency and clinic/Department orientation of new employees
  • Special projects as assigned by the Program Director, and other executive staff
  • Identifies and interviews candidates for vacant positions.
  • Participates in Managers’ meetings
  • Participates in relevant internal and external training.
  • Participates in monthly DOH and Health Homes Network meetings.
  • Performs other related duties, as assigned.

Minimum Requirements

 

Minimum Education:

  • Masters degree in Social Work preferred or Masters in other social service field with commensurate clinical training and experience.

Knowledge, Skills and Abilities Required:

  • Two (2) years of supervisory experience
  • Ability to manage, supervise and provide leadership for up to 12 staff members.
  • Demonstrate ability to organize, develop and implement program standards.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills.
  • Ability to work in a team management system.
  • Good verbal and written communication skills.
  • Computer literate and working knowledge of Microsoft Office (Word, Excel) and Electronic Health Record Systems.
  • Good interpersonal skills and ability/interest in working as part of a team.
  • Knowledge of HIV/AIDS, chronic medical conditions, mental illness, substance use, and homelessness.
  • Knowledge of City, State, and Federal entitlement systems is helpful.
  • Bilingual English/Spanish Preferred.
