PRIMARY PURPOSE or JOB SUMMARY

 

To perform all of Community Services Block Grant (CSBG); General Energy Assistance Program, Case Management, Youth Leadership Program; and Low-Income Home Energy Assistance Programs (LIHEAP): Direct Assistant-Heating and Cooling; Emergency Crisis Intervention Programs – Heating and Cooling Program activities at an assigned county.  

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

 

 Allocation of Time for Non-Administrative Items 1-10: CSBG 60%; LIHEAP 35%

 

  1. Answer client’s questions concerning program services.

 

  1. Set up your own schedule of appointments with clients.

 

  1. Interview and assess applicants for eligibility to programs based on criteria set

forth by the funding source. Case Manager may be personally, financially responsible for failure to follow criteria set forth by funding source, as determined by the Program Director and Chief Executive Officer.

 

  1. Complete all proper documentations as outlined by the funding sources to be included in the client’s files (application, client profile, award, etc.) as well as imputing the information into the DBA FACSPro System.

 

  1. Make referrals to other social services providers; help the client make contact with appropriate services, follow-up, etc.

 

  1. Conduct home visit if needed, to ensure goals and objectives of the program are being met.

 

  1. Represent the agency within the community, attend meetings, speak at functions, etc., to communicate the CSBG’s purpose and progress, and to be an advocate for clients and programs.

 

  1. Act as advocate between the client and the vendors. Follow up with the vendors to ensure that the process is completed.

 

  1.  Attend scheduled staff/program meetings.

 

  1. Participate in trainings provided by the agency and/or other entities for personal and professional development

 

Allocation of Time for Administrative Items 12-19: CSBG 3%; LIHEAP 2%

 

  1. Perform administrative duties and keep accurate documentation of all services provided to clients.  Prepare reports as requested.

 

  1. Participate in the annual compilation of information for the Community Needs  

Assessment.

 

  1. Participate in the Agency’s internal monitoring of files and systems.

  1. Maintain daily logs to ensure that program funds are in balance with the DBA FACSPro System.

 

 

  1. Participate in trainings provided by the agency and/or other entities for personal and professional development.

  1. Perform other job-related duties as required.

 

ACCOUNTABILITY

 

For meeting the objectives of an assigned work plan and maintaining the integrity of the CSBG/LIHEAP program.

 

SUPERVISORY RESPONSIBILITIES

 

None

 

 

 

QUALIFICATIONS

 

BS/BA Degree in Sociology, Psychology, Social Work, or related field, or a combination of education and experience commensurate with the job description.

 

Minimum 3-5 years experience in a human service environment working with low-income individuals.

 

Valid South Carolina Driver’s License

 

EQUIPMENT AND MACHINES

 

Personal computer.

 

WORKING CONDITIONS

 

10% of work time is spent in the field.
